And rather than being entirely random, it might be more interesting and consistent if a transition were triggered by stress or intense emotion or something else vague that tends toward public situations. And ideally the treatment itself could be justified more directly than "smearing mucus over the intestines", perhaps more like "carefully extracting the corrupted emots (or the 'emotic energy' they're made of).
