Maybe she comes back to her exam room and finds her clothes are missing because a cleaner came by and assumed they were forgotten, or she gets lost and can't find her room with her clothes.
Maybe the doctor insists on a nude weigh in and the scale is in a hallway.
Or maybe she's brought to the doctor for a routine checkup with a friend and the friend stays in the room for the checkup and refuses to leave.
Or maybe she's at a school nurse and a classmate comes by to deliver a message while she's in the middle of an examination.

I mean even though I hate not wearing underwear there's been a few times when I changed into a swimsuit before going to the pool and just wore it under my clothes on the way there, only to discover I forgot underwear much to my horror, and I was forced to go commando on my way home because I couldn't wear a wet swimsuit under my clothes. It's an easy mistake to make, and could definitely lead to some ENF situations if a girl did it.
