The only time it really hurts in my experience is when you're walking on actual rocks, or some prickly thing in the grass. Neither of which really come into play in ENF stories (at least not in the ones I've read).
Well, there was that one time in TAG where the main girl got scratched by a bush, but that was mostly to drive the point home on just how naked and vulnerable she was.
