I also think something like accidentally getting naked in a mixed change room/spa/sauna and then being seen by the opposite gender could work, but maybe that's covered by Shared/Mixed Bath.
Medical examination is there, but along those lines, I don't know what I would call it, but basically someone you know seeing you naked.
Something that could take a situation like a medical exam where it's only mildly embarrassing because it's a professional, but it becomes extremely embarrassing when it's someone you know that you might see again or day-to-day or that's a part of the same friend group, i.e. a former classmate, friend, neighbor, coworker, in-law, etc.
Duty/obeying orders might be a good one as well, like a female in a military where she's running late and is in the shower, so she doesn't have time to get her uniform on and is lined up and at attention naked, and she's forced to go about the rest of her day naked because the leader doesn't dismiss her to get dressed.
