How about 'builds' then? To give the game replay value artificially. All the cool kids are doing it. People nowadays are suckers for RPG elements in everything. Level up, get points, buy goodies. You already have a score system set up.
For example, at some point in the story, you can make the girl decide what kind of exhibitionist she wants to become. Let's call them Stealthy Exhibitionist and Shameless Exhibitionist.
The Shameless Exhibitionist would buy upgrades that would increase her speed (but increase 'noise' being spotted from farther), her Confidence Bar to tolerate being seen for longer, recover Confidence faster and maybe a bonus to masturbating. Not being seen with her would be really hard so some challenges would be out of her reach.
The Stealthy Exhibitionist could buy upgrades like reduced noise while running, maybe ability to crouch, people needing to be closer to her for spotting it and stuff like that. She'd have a comparatively easier time not being found but if she's seen even once it's game over; she could even be the hard mode.
Just brainstorming random stuff that comes to mind. Just saying that the Shameless Exhibitionist could be when the girl who decides she secretly wants to be found and enjoy herself while the Stealthy Exhibitionist could be the 'ENF Route'. They could both play the story exactly the same except for a few lines of text difference between each of the 'builds' denoting the different personalities.
Again, just putting a random example; and what's more important here is that you realize your vision to keep you inspired to keep working on the game. it doesn't have to be builds, it doesn't have to be those two, it doesn't have to be only two. Then again, maybe it's just bad advice and you should ignore it.
