Depending on how it works though, its not like there are set rules its all very vague but given how it seems to be then if they're in their transformed state then no. They would look like a nude Sailor Moon to people not a nude Usagi. Unless the hidden identity is tied to the tiara or choker and taking that off does it but who knows. Transforming also works as make-up though and appears to add it to their face some maybe that works as a magic masking layer over their face too, and if it can't be rubbed off like normal make-up then they'd have to de-transform to be recognizable again.

They'll be like "yeah he does have the same height and chin now that you mention it", for the sailors people would prob say "well she is a blonde teenage girl too and I guess she does sort of look like her now that you mention it". Either its one of the accessories that hides their identity or magic make-up or a magic aura effect that works so long as they're in the transformed state.
