I'd argue there's a method and tact to how fucked up Metamorphosis is. Many other doujins make the demoralization happen in a few pages, but the whole story shows the slow burning, excruciating process of a girl fucking up her life for acceptance. I've also seen more fucked up doujins but they're just outrageous and goofy. I think similarly disturbing hentai like Geiger Counter are popular for the same reason.
