I dunno if anyone was interested... but I decided to do more research into how Dorothy's heart monitor works for anyone who is interested or needs references.
Basically, the monitor which she straps to her waist or coat is the only external part, and monitors her heart for any potentially dangerous arrhythmias. There is a wire coming out the back of it that goes under the clothes and connects to the vest. The purple circles on the vest are not actually purple or visible from the outside, but they function as the electrodes that monitor the heart rhythm. The two rectangles on the back are the actual defibrillators that send the shock to the heart when needed. There is a layer of gel in them to prevent burns to her skin.
She has to wear the vest and monitor at all times, even when sleeping to keep her safe. She only takes it off when showering and bathing. She is very diligent in wearing it, but she is also pretty self conscious about it, worrying that it might make her look ugly or weird.
Someone mentioned that as she gets older she could wear the monitor under her clothes, but looking it up they look too bulky to sit under the clothes properly. I could see her potentially getting an ICD defibrillator, which is a smaller one surgically implanted into the chest when she is older, but I am not certain.
