one of the problems is mentality, another major one is the lack of public transport in the united states. cycling your entire daily commute isn't the greatest thing for most people, but cycling a few kilometres to a train station is a lot more manageable.
urban areas just happen to have more incentives for people to use bicycles, including public transport. add to that the fact that some people are simply forced to ride bicycles in big cities.
remember that the reason the concept of suburbs exists isn't because of the automobile, it is because of railways
