Other then that they definitely were lucky the plug flew out at only 16,000 fts (any pilot in here with a ballpark estimate of what the corresponding cabin altitude would have been?), I'd expect a few more injuries and/or frostbites had this happened at cruising altitude.

Just look at how the BEA (French NTSB) dealt with Concorde and the early 320 accidents, the national carrier and builders never did anything wrong apparently.
