Two tail lights about two feet apart (so like, one on your seat stem and one hanging somewhere on the end of your rack), one steady and one pulsing. This will maximize the ability of drivers to both see and track you.
Reflectors are great and you should have dozens but they're not a replacement for active lighting, after all they depend entirely on the driver to not have his own lights burnt out and waiting for repairs.
Wear the dang yellow vest. They're cheap, and yellow really pops at night.

Flashing is just a literal eyesore and will make people try to run you off the road that much more.
Headlamp on the helmet both helps you see where you're looking and makes you look more like a human and less like a motorcycle.
