When riding in the tops of the dropbars while climbing, shifting down was an issue because one hand needs to go down to the bar end to shift so you lose some power when you're out of the saddle and pushing on the bars.
I can't think of anything pretty... You could put on a second stem and bar to create a bumper in front of your levers.
There was also the "Kelly rake-off" thumb shifter mount. Dia Compe has another strange option.. ene wing shifter, but I don't think either could be found cheap.
You can mount a thumbie next to your stem with Paul thumbie mount. Or set up the bars so that your main riding position is in the drops with the bar end reachable at all times. Pic related seems set up that way with Kelly takeoff style shifters
