My rear tire came loose after a test ride with a new chain.
Gearing and coaster brakes worked and suddenly the rear bolts came loose when i applied the coaster brakes, and the tire touched the frame.
Happened before i changed chain when i first got the bike as well.
(only then it had a shorter chain and the tire was further towards the seat-post)
Also the attachment point for the gear cable twisted around as seen in the picture.
