In June 1998, a flatbed semi truck hauling rolls of coiled steel was hit at a grade crossing by a South Shore Line commuter train in Portage, Indiana (Chicago area). The chain securing one of the steel rolls broke and it entered the cabin of the train, killing 3 people. Two died instantly, the third lived almost 45 minutes pinned under the steel before succumbing.
