you go over a converging switchpoint (from far to near in the photo, on the right track) that is incorrectly switched. in this case on the photo it would be the nearest switch turned the opposite way. this causes it to leave a gap on both sides due to the weight of the train pushing it apart. this doesnt actually cause any problems because of the direction you are traveling. you cant derail from that.
the problem is when you put the engine in reverse. this causes bogies to "randomly" switch to left or right track as you back up. one bogie goes left, one right. you realize someone fucked up but its too late. youre now multi track drifting
