if ever you want death defying speeds one of the brake set ups for short handle bars is inverted bar end brake levers,
more common on motorcycles but sometimes seen on bikes till about 1950 ish.
there are some for modern handle bars like DiaCompe 188 (inner diameter between 18 or 20 mm) but most are for road bike handle bars

as the other reply says some handle bars dont have much room around the hand grips
so there is only room for a brake lever, and integrated shifters may not fit properly in the curve of the bar
some old bikes and motor cycles with very short grips use an Inverted bar end lever to free up space
