That means it's had a front on collision.
It's one of the main things to look out for on old bikes. Often it will bend the frame as well and you'll see rippling on the paint in 2 spots near the headtube.
Also if you look on pic related, here are some tells for a shitty old road bike. The brakes have those extension levers 'suicide levers' and no rubber hoods on them. The shifters are clamped on the stem, and not located on the downtube of the frame. There is a kickstand. There is a metal plate behind the freewheel. And the wheels are 27" steel, not 700c aluminium. That is a big one because steel rims have abysmal braking especially in the wet and tyres for that size are quite rare.
