aero levers pull the cable out rather than up which gives them more leverage.
That's why they caught on. At the start they were considered garish and annoying to work on but the increased power was undeniable. That's the reason it's so stupid to use them, especially with cantilever brakes supposedly being ridden offroad and touring, which need all the power they can get. I mean i have some on a beater but on a $4000 'serious' bike is just bananas.
