I think it's interesting you have ass AND dick pain, as if it were one or the other, there would be an obvious move. I'd try something with a center cutout, maybe one step wider. I'd also try one the same size as the tioga, but curved, like say the Selle SMP.
San marco might be a good idea as well.
If all of these don't work at all, maybe try something completely different like say, the ISM PM.
