The spindle should line up with your 5th metatarsal, closer to the arch than was previously believed, but still pretty far forward of it. If you're riding with the spindle in your arch, you're not getting as much power out of your pedalstroke as you could, clipless or not.
Manufacturers are starting to catch up and a lot of cleat interfaces on the most recent shoes available are set about 7-8mm further back than the previous version to allow for more adjustment.
