or your crank arms have come loose and deformed the taper (unlikely seeing as it's both)
if it's the former, remove the crank and replace your bottom bracket. New cartridge units that just go straight in are cheap.
if it's the latter, replace your crank
is it actually square taper like in your picture? Then you need this to remove the crank. Or if the crank arms are loose you can just remove the bolt and they'll fall off.
