The derailleur (and thus cage) is brand new and doesn't seem bent. The hanger is a stamped integrated frame piece
I pulled the cable to remove slack upon tightening it down, but didn't pull with all my strength
B screw I'm not sure because the derailleur manual suggests as little space between pulley and cassette without touching, but resources like Park say ~5mm. I went with the manual.
Everything is tightened to spec via torque wrench.
I mentioned it before, but it's all new parts. I did a 5 mile test ride yesterday, but that's it. I keep suspecting it's the wheel not being totally straight in the horizontal dropouts, but adjusting it as close as possible to straight doesn't seem to do much. I've never had this much trouble with shifting and it's driving me crazy. I'll check out Moustache Man's videos again to see if there's anything I missed.
