Those within the city do one of the following:
1) Subway
2) Local bus to subway (mostly in the outer boroughs)
3) Local bus to ferry to subway (Staten Island)
4) Express bus (Staten Island and some more well-off areas near the city line)
For those coming from beyond the city limits, most people use commuter rail or the PATH, though there is a very large commuter bus network to points west of the Hudson river.
Also, the extremely poor outside the city take a local suburban bus to the subway, and there's enough of them that a few routes run 24/7.
