You pretty much nailed it. I rode delivery and no matter what you start with it just slowly morphs into a fixed gear, because shit breaks and you don't want to spend money.

you are also assuming that your cog and chainring is perfectly round, and that you regularly adjust your rear wheel to account for chain stretch. Even then, I'm sure that things may cause your chain to drop

The chain of a fixie sees stress in both directions unlike freewheeled bikes so setting proper chain tension is extremely important in preventing a dropped chain. Dropped chains on a fixie spells disaster since it locks the wheel. Honestly though, I never had a dropped chain since I regularly measure my chain tension which is at ±1cm
