Only one removal failed, there was a commie bitch called Marta Ugarte who wasn't completely dead so the captain ordered his men to untie a bit of the wire they used to tie her to a piece of rail and strangle her with it. When they tied it again, they didn't tighten it enough, so when they threw her into the sea her body went afloat and ended up on the beach.
The one you posted is newer though, it's an Eurocopter AS 532 Cougar from 2010, a newer version of the Aérospatiale SA 330 Puma choppers they actually used back in the day.
