With a road bike fit there will be a bend in your knee throughout each rotation but this bend does not seem to significantly alleviate pressure from the perineum as the foot travels along the axle of rotation (bottom bracket). The trend in modern road bike geometries (i.e. top tubes rising as they arrive at the steering tube) suggest that the whole saddle above the handlebar thing is pretty fucked up are looking for ways to provide higher handlebar height options while still being aero. .
