Ok, so what about the loses at a high intensity? I don't subscribe to spin to win or mash to smash but there's a sweet spot that's somewhere between mashing and spinning for max efficiency.

Efficiency isn't everything. At a high cadence I never have a problem with lactic acid buildup. Sure my heart feels like its going to explode, but my legs are fine.

The average cyclist is doing 50w going down the MUP. Bikes are pretty efficient. If you got a power meter I'm sure you'd be surprised at your power output.

I just calculated it:
At 50W I would go about 18,4 km/h, pretty slow indeed, but not so slow that stability becomes an issue.

if that's right then your average commuter puts out 50w on the flats because 10mph is about the standard measuring stick for commuting (including the occasional stoplight 18.4 kmh/11.4 mph seems right on)

Your average commuter doesn't ride a roadbike, weights at least 20 kg more and is about 15-20 cm taller.
So probably closer to 70-100 W.

maybe it's because my total revolutions are higher but if I spin vs going normal or mashing then my knees get sore.

eh, I'm working with about half a meniscus on the right side. While I don't doubt a little front-back slide might help in the end it's not going to make a big distance.

The big key is taking NSAIDS after the ride, taking an immediate shower and icing any swelling on the right knee.

Yea, that is probably the reason why low candence is more efficient.
I tend to hover around 100-120 rpm at threshold for lower muscle fatique, higher when climbing hard or sprinting...

Dude, are you running a 34-42 low gear or something?
I can understand if you have really light shoes and very short cranks, I guess.
Climbing seated is the pits man, I smash high 60 rpm standing and about 100 climbing if I have the gears, but once I get to 4% or so standing up is way better feeling and more efficient.
Then again, I have full size chainring and a short cage RD so gearing is long for climbing.

My shoes are not light at all, but my cranks are short.

Doesn't work for long climbs and 4% is nothing.

That might be your issue, I run a compact.
