Affirmative action is actually occuring in the aviation industry, but it is done to an extent. Basically, they're getting as many women to perform the aptitude tests as possible, in order to find more women who can actually fly competently (under the understanding that there's probably a lot of women around who maybe haven't considered a career in flying, even though they're likely highly capable of doing so).
If you believe in the Swiss Cheese Model of Accident Causation, then I've got to ask why we haven't seen someone slip through not only getting a pilots licence from an aviation authority, but also passing the airline tests and the type rating to only make a dangerous mistake?
