they have metal in them but only in a few places
one of the first kind I replaced with shimano m system cantilevers, they worked much better

I have noticed that v brakes all have very slightly different arm lengths
of course the longer you go the more overpowered and spongy the brakes get
some of the older ones have much thicker arms with stronger metal but that just moves the flex problem onto the brake bosses even harder
the largest v brake is on the honda rc115
and some people have made custom ones for their motor bikes

on shorter runs you can split the cable in two closer to the lever, fine for the front brake
another use for cable splitters is in down hill racers
with four wheels the brake can be set up for brake steering .eg each lever controls two brakes on one side
or just the normal front to rear bias with one lever to do each axle
tadpole trikes need no rear brake and are set up with one lever per brake
