With nuclear, it literally takes one of the more severe natural disasters the world has seen in modern times, combined with a 50 year old reactor design and a long chain of individual failures where if any single one of them was prevented, the entire disaster would have been prevented.
Reactor design has come a long way since the '60s, and is far safer today. The old reactors are already the safest scaleable form of energy we have, seriously claiming that they're too dangerous is just uneducated fearmongering.
