Outward opening cargo doors in and of themselves were pretty unavoidable (the 747 and L1011 also had them), MD just took a lax approach to making sure they would close safely because a re-design would be too expensive. The tailplane design was less than ideal, but could be fixed by placing some cutoff valves so that an explosion would only take out the tail controls. Multiple DC-10s blew cargo doors, but only one blew its hydraulics.
