Modern elevators are pretty well impervious to any single thing causing a fatal accident, and most of my repairs come with an explanation of how the problem occurred and what faults the system is reporting. If a fatal accident related to the functioning of the elevator ever did happen, it would be on the building management or their adjuster, and there would be a massive investigation. People would go to jail.
Typically, the entire elevator locks up when something I work on breaks down. People are always able to be extracted, so don't worry if you do get stuck. If the power is out and the phone isn't working, pull out your cellphone and call the fire department. If the guy responsible for getting you help somehow hasn't noticed what's going on, the fire department will take care of you.
