It should be tight enough so that there is no play and you can't turn the spacers or bearing dust cover at least with only your fingers, though full palm grip is my way of checking to be sure. Make sure you are tightening it while the wheel is off the ground (hold the bike by the frame or mount it on a hanger rack), with some fork geometries, you won't be able to pre-load the bearing properly, if the weight rests on the wheel.
This is just a guess, since I'm describing my own problem from a few months back. It might well be anything, even the threads on the handlebar mount clicking cause they get loose. Post a more detailed description maybe?
