A second cab adds size, weight, more stuff to repair, and more cost and it's not necessary because most American commuter railroads use passenger cars with cabs to remote control the locomotive from the other end. (Here's a cab car from my local railroad, ugly as fuck imo.) America also has a lot more places to turn locomotives around when necessary, but that's usually only done occasionally to balance out wear on the wheels etc.
Some American railroads are slowly starting to modernize and adopt European stuff too, there's at least one commuter railroad using Stadler FLIRT variants and several more have ordered them, and Caltrain is replacing its diesel locos and traditional passenger cars with Stadler KISS EMUs. Amtrak also uses double-ended Siemens electric locomotives for commuter services around New York.
