That last cable bend looks a bit compressed. Have you checked for smooth shift cable travel through the outer cable sleeves?

Also you might need to invest in a derailleur hanger alingment tool, make one yourself or have the local bike shop verify that your derailleur hanger is properly aligned.

What do you mean? It's pretty clean.
Yeah it's not "I use brake cleaner on my drivetrain" clean. But it doesn't have to be.
Also, I use this bike in the woods weekly year round. Most of the time I only clean the drivetrain, not the frame. Why bother?
