Great Lakes tragedy, but I wouldn't call it the worst marine incident in history. Plenty of other shipwrecks that were worse, either in human lives, financial disruptions, or other reasons.

I'd go for bad design, the length of the vessel is a contributing factor, with both ends sitting on crests and the middle suspended above a trough. Now, this may have been a one off encounter, but it meant that in such a situation, a structural failure was inevitable. There are vessels longer than this, but the bulk and size proportionally would have added strength to the design as a whole, a skinny narrow vessel like the Fitzgerald would likely have a lesser chance of coming out of that.

Read about those. The General Slocum one was unfortunate, but it was silly, again the usual overcrowding and lax attitudes. Sultana was ignorance in the extreme. They were forewarned and did nothing. Can't say that they deserved it. But it does mirror in many respects some of these modern ferry accidents in the 3rd World.

It was a lake ship. For use in a lake. It wasn't designed to handle ocean-tier seas, because lake. Like all those Mediterranean or Mexican gulf cruise ships, take any of them out of their designed waters & into open ocean & they'd immediately break up & sink like Edmund Fitzgerald. Freak storm hits the big American lake causing ocean-tier conditions, boat sails anyway because "muh profits", everyone dies and their bodies fail to decompose so there's a bunch of spoopy corpses floating about innawreck to spoop out any divers that go down there.

The open loop scrubbers I've worked with all had filters to clean the used washwater before pumping it overboard. It's also diluted in a static mixer until the pH is above 6 (IIRC) before going overboard.
