Normally how they fail is, due to poor installation, they move up the taper, lose preload, worm back down, destroy the taper on the crank, then are irreparably loose. But that's not catastrophic. Sure if you don't have a cap on the bolt might unthread and fall out and then the crank fall off but the arm will be obviously loose for a long time before then and it's only going to bite you if you're retarded.
