I see 2 solutions:

Making a really good test that would actually test one’s ability to drive safely would be not only hard to construct, it would be expensive. Things like skid pads, closed tracks, long testing times - it costs money. It would prohibit many poor, but skilled people from driving - especially as it would need to be redone regularly, as people can lose their abilities (reflexes, eyesight) as they age.

Killing someone with a car should be treated as a regular murder - and regardless of how it is punished where you live, I think it deserves death penalty, in most cases a painful death. Causing any collision, visibly losing control over your car or things like running a red light, overtaking when it’s not allowed, should end with lifelong revoking of the license (and public beating). Stuff like not using indicators should guarantee you a beating lighter and losing license for maybe a few months. DUI should be punished by being covered in ethanol (best thickened with something so more sticks to body) and lit on fire. I believe that after a first few convictions, he road situations would improve greatly.
