How do you solve this dilemma? How does the train tunnel move forward when it's blocked by building foundations?

Go around or under it. In some cases you may be able to go between the piles or the foundation was designed in a manner that could accommodate a rail line later.

>Build around it
>Strengthening/redistributing foundation of building to allow space
All three are expensive, and depending on the project (route planning, building size/value, budget, political weight, etc) one may be the most obvious choice.
