I frontload almost entirely as you can see in the above pic with no problems. Had 18kg packed on there at several times. I also don't see how torsion would cause this? There's nothing twisting at all.

Impotence has to do with pressure on that area (perineum?). Regardless, you're right about standing up occasionally is very important for blood flow and stretching/using different muscles.
I'd recommend doing some shorter tours, maybe week long or so to get a feel for it before setting out on a super long one.
