Getting lost happens to even the best UE (Urban Explorer), but there are things that can be done to help lower the chances of it happening. If possible travel with a partner, keeping groups small. If you do have a friend who's going in with you I would get a small, cheap radio and throatmic for easy communication. Look into the Baofeng UV-5R on amazon. At the very least if you are going in with someone besides yourself have a walkie and decide on a channel before your operation. Carry a small medical kit in your bag, it really can help out. Things are sharp, rusty, stabby and unpleasant. Especially so in the dark.
Patrols will happen if your building is still being monitored. Make sure you keep your flashlights low and to the ground that way you minimize your detection. And keep it quiet! I don't care how alone you think you are, until you've gone there over five times and know that no one will appear keep it quiet.
As for getting stuck it has never been an issue except when I explored the underground steam tunnels under MIT. Those things were a labyrinth. If you need to mark your path. Reflective tape can be a life saver but the best advice I can give is to be observant. I have a photographic memory so I build myself a mental map of wherever I go. Just keep calm.
