payload distribution doesn't seem to be a problem in helicopter as their fuselage is usually horizontal so sudden weight shifting doesn't happen to often and their stability can counteract this, unlike fixed wing aircraft. as long as the payload is close to the center of the rotor and it doesn't swing violently everything should be fine, especially with weight well under its maximum payload capacity
the causes can be pilot error, technical problem (caused by poor maintenance, old airframe or just stuff breaking), weather (gusts of wind aren't uncommon in the mountains) or a combination these factor.
