The first thing you want to do us make sure you're underground, as your main threat of radiation quickly gets blocked by the earth around you. This is a trade off, however, as you will need an air filtration system but we'll get to that. Next you want to line the subfloor above your head with radiation blocking material. To half the radiation you'll need:

I'd recommend treated wood and a couple sheets of metal in the subfloor, as much as your joists can support. Next you're going to seal up your shelter, making it air-tight, 99% of conditioned air is lost through 1% of the house, anon, so be thorough because radiation sickness is a bad way to go. Install an air filter, or make your own. All you really need is a heavy-duty air filter with some extra filters you really just need to buy, no DIY is good enough for how important your air filter is. As for your water, you need to set up a reverse-osmosis system to get the radiation particles out. I'll leave you to research that on your own.
Honestly, besides losing about a foot of head-space in my basement, no one would even guess it's a fallout shelter.
