A small percentage of land requires a permit, either paid for or drawn in a lottery, to camp over night because they are deemed particularly delicate or dangerous environments and they don't want mobs of people shitting up the place. An example would be the Enchantments which is part of the Alpine Lakes Wilderness in Snoqualmie National Forest. Anyone can day hike the area, but a limited number of overnight camping permits are lotteried each year because they want to balance public access with preservation of the area.
As for rules, each individual USFS or BLM district will have rules for the land they manage. Examples would be limited or no ORV access, no open fires during dry season or above certain elevations, no pack animal grazing, etc.
