A bent shopping rewards card with a bit of duct tape to adjust thickness and fit will take care of that problem cheaply and easily.
It will greatly reduce any rattle and improve the retention and fit of the knife...I've also used the plastic from a 2 liter soda bottle to do the same thing...with my other knives, just didn't have one handy at the time.
No clue what to do about not slicing the snap strap other than just being more mindful...shit happens.
