I need that extra half size because the width isn't great enough in a 10.5. With boots I also go 11 because I wear thicker socks underneath and tight socks/boots are the quickest way to get toe and nail problems in under 24 hours.
Unfortunately most companies don't offer different widths in boots, I only seem to find that in dress shoes and golf shoes where I get an "E" width. If you're shopping for footwear always check though, some companies do periodically offer it like Vasque who give you the choice of medium or wide.
I fucked up my footsies years ago wearing tight boots and wouldn't wish it on anyone, it's like 6-8 months of recovery if you get surgery so you can write off an entire season of going /out/.
