I can only think of a handful times I went into a tight passage in a belly crawl that had a dead end and I had to back out of (there is one in particular that I'm not sure if it was actually a dead end and suspect there was more passage, but it was already a tight situation and I didn't want to end up like OP's pic so I just bailed). Most caves have been pretty well surveyed, and I've only found a couple of passages that weren't marked on the maps that I had (and they were small bypasses). Safe is relative to your skill level and your gear.
