The guys in the article I linked had to fall back on drinking blood from the birds they caught when there wasn't enough rainfall, which doesn't sound very pleasant. Medical services, I'd go into something like this accepting that if I get seriously sick or injured then I'm probably going to die.
Assuming you have some way to dig, apparently big enough islands can retain rainfall underground (pic related).
