Except he didn't use an overhand onna bight, he used A FUCKING SLIPKNOT, and this one time he didn't clip the sling back to his harness "because it was just a single pitch rap." When he went on rappel the tail slipped through and the sling completely came off his biner, leaving him hanging solely by his prusik backup.
Apparently this is the way he's always done it, and he's never noticed this because he's always clipped the sling back.
