My idea was to mimic a mummy bag so there isn't any wasted space in the corners, and so the foot area has enough space to feel comfortable. The alternative would be to just sew two pieces of fabric together like a taco, and there'd be a little nipple at the top where your feet couldn't access, and which would get cold. It's not required for a quilt, but not too hard to add.
